feat(agents): create agents from a description, with profiles - #605

Add optional appearance color to the agent schema (mapped to existing chart tokens, stable derived default), an AgentAvatar component, and library helpers for blank creation, removal and reference lookup.
New agent dialog (blank or template) and palette command, a sectioned editor, a profile header, and a delete dialog that blocks on playbook or project references. Show the avatar in the list, sidebar, Kickoff Who, Fleet badge and Flow node; extend the dev preview with new/edit/delete.
Cover schema appearance, the appearance helpers, removeCustomAgent, blankCustomAgent, reference lookup and the sectioned editor render.

Summary
Custom agents can be created from a one-line description, started blank or copied, and each agent gets a profile. The editor shows only what most agents need and folds the rest.
