Summary

Weekly queue reconciliation for 2026-10-05 (SAM task 01M45AG431MM1A1ERVN3PJ2HZK). The task records in tasks/active/ and tasks/backlog/ were checked against main at ee80b0ee0 and against read-only production state, and the five open PRs older than seven days each got a status comment. The SAM memory half of the task (knowledge, ideas, policies) was done through SAM MCP tools and is summarized in the ledger, not in this diff.

The brief estimated about 75 active and 260 backlog files; last week's run (#2198) had already cut the queue, so this run is a delta reconciliation against the 26 PRs merged since 2026-09-30. Production runs main HEAD ee80b0ee0 (Deploy Production run 37245799681), so all 26 are live. The full per-file evidence is in the ledger, tasks/archive/2026-10-05-weekly-queue-reconciliation.md.

    • 2026-03-29-vm-agent-read-header-timeout (superseded, deleted): Complete ACP interactivity and distribute the verified Codex runtime #2217 (6a042d599) gave the upload handler its own read deadline from FILE_UPLOAD_TIMEOUT (packages/vm-agent/internal/server/file_transfer.go:83-91, real-socket test), and the short server ReadTimeout is kept on purpose for every other route.
    • 2026-07-11-codex-staging-oauth-refresh-token-revoked (obsolete, archived): fresh openai-codex VM and Instant turns completed on staging on 2026-10-03 for the same smoke user. Archived instead of deleted because tasks/archive/2026-06-15-codex-acp-midprompt-disconnect.md links to it (repointed).
  • Narrowed (8): a Reconciliation 2026-10-05 block saying what shipped this week, with evidence, and what is still open.
  • Corrected or extended (19): a dated note with a corrected file pointer, a corrected premise, or a new finding. Bodies were not rewritten (prettier normalized one emphasis marker in a file it formatted) and no checkbox was changed.
  • New entry (1): 2026-10-05-sleeping-task-status-follow-ups, for the fix: stable task identity across sleep/wake cycles #2230 regressions below.

Found during the audit (verified in code and read-only production D1)

  • fix(api): truthful task-recovery liveness records; bound ceiling sleep deferral #2222 shipped a production bug that fires every five minutes. The stuck-task dedupe binds a LIKE pattern of 51 to 69 bytes, over D1's 50-byte limit, so each sweep writes a duplicate platform_errors row (59 rows for one task since 01:36Z today). Recorded on 2026-08-07-fix-stuck-task-sweep-pattern-complexity.
  • fix: stable task identity across sleep/wake cycles #2230's new sleeping task status is missing from the "active" status sets. Slept VM conversations vanish from the dashboard's Active Tasks, list_project_agents and the account map; send_message_to_subtask, send_durable_message and stop_subtask refuse a slept VM agent; and, from code reading, a slept VM chat probably renders as provisioning. New backlog entry.
  • GPT-6.1 Sol still fails inside SAM with HTTP 400 "not supported when using Codex with a ChatGPT account". SAM pins Codex CLI 0.156.1 (with the SAM C2 patch). Tracked as SAM idea 01M45CXE5ZG10WTT30V5HCSY9V.
  • An agent auto-commit reverted a deliberate config change on main: .codex/config.toml model_reasoning_effort is back to "low" (c9316fb3f, carried in by Complete ACP interactivity and distribute the verified Codex runtime #2217, undid 2f78efcff).

Open PRs older than seven days

Commented on #1788, #1817, #2020, #2062 and #2160 with what changed since 2026-09-30 (only drift: 142 more commits on main) and the one decision that ends each park. #2160 aged into scope this week. Links are in the ledger.

Each file's work is merged and live: production runs main HEAD ee80b0e
(Deploy Production 37245799681). Each archived file gets a provenance
footer naming its landing PR, merge commit and first successful production
deploy; boxes stay unticked unless evidence exists. The ProjectData
capacity emergency stays active with a 2026-10-05 status block (9.72 GB
and falling, breaker closed, 0 failed or poisoned migrations).

Delta audit of all 214 backlog files against the 26 PRs merged since
2026-09-30. Adds a dated block to 25 files (8 with new progress, 17 with a
corrected fact or new finding), deletes one superseded entry, archives one
obsolete entry that an archived task still links to (link repointed), and
files the #2230 sleeping-status gaps as a new entry.

The sparkline note cited FailureCard.tsx lines that use the
--sam-color-fg-secondary custom property, not the text-fg-secondary class;
separate the two. Add pointer-correction notes to the two recovery and
sleep entries whose citations #2223 and #2230 moved (one pointed past the
end of the rewritten session-recovery.ts).
