Preserve repository-grounded review findings

Postil now reviews large changes and repository-dependent claims without treating the patch as the evidence boundary, while preserving unresolved findings and binding every repository lookup to the reviewed Git object. It also grounds temporal claims in the request date, adjudicates absence and duplicate claims across the complete source corpus, and keeps provider outages distinct from invalid model output; the default and qualification matrices plus the 70-case screen pass for the v0.8.13 candidate.
