Upsert support

Hello,

I'm curious about support for upsert - particularly with postgres, i.e., INSERT ... ON CONFLICT ... DO UPDATE (set specific columns).

I noticed a PR which might be relevant, and mentions an insert_or_update function, but could not find this function in the code or in the documentation.

You are right...insert_or_replace is not documented anywhere, which is weird. I was very certain we did document that.

In any case, it is there, just scroll to the bottom of this header file. I will add documentation shortly.

Does insert_or_replace support selecting specific columns in the ON CONFLICT clause?

Here's an example (in python's sqlalchemy) of what I'd like to be able to replicate in sqlgen.

@andresenwc this is currently unsupported, but I see how this is useful. Could you open a separate issue?
