[codex] Add puppet table tests

Summary

• add first tests for Puppet info, logs, state, and facts tables
• make YAML and facts loading injectable for deterministic coverage
• cover YAML read/parse errors and Puppet facts command/JSON errors
• fix transaction_uuid output mapping in PuppetInfoGenerate
