Keep claude running between questions - #80

One claude process per shell (stream-json in and out, behind a unix socket) answers each question without claude's ~2s startup, and holds the conversation. Restarts with --resume after a cd, interrupts the turn on Ctrl+C, stops after preheat.claude_idle_minutes (default 15, 0 = off), on /new, tool set, and shell exit. Falls back to per-question claude -p when it cannot take a question.
