Work that ends needing a person or failing was only announced by in-app toasts while the window was visible, so it went unnoticed in the background.
The desktop main process now watches the local service and sends a macOS notification when an activity newly enters
action_requiredorfailed, with no backlog on startup and no duplicates. Clicking opens that activity. Notifications are suppressed while the window is focused and can be turned off in Settings. They stop when the app quits.Validation:
pnpm checkpasses (407 tests). Real notification display was not checked.
