docs: logo and badges on every package README

Summary

Only the root README and pgpm-modules/dcrypt-vault had the logo header. This adds the same centered logo + CI badge + npm version badge block (the dcrypt-vault pattern) under the title of all 14 packages/*/README.md, each npm badge pointing at that package's own package.json.
