Fixes the four-way comparison success criterion so honest blocked output can be correctly calibrated. Adds concept-based KPI alignment, ambiguous-mode handling, a pinned invocation prompt and clarification policy, host reproducibility metadata, and a separate human approval receipt that avoids self-referential adjudication hashes. Candidate baselines remain unapproved and exit-gate metrics are unchanged. Verification: node evaluation/semantic-assurance/evaluate.mjs and npm run check:all.
