- Add quantum.dynamic.alloc op with builder and verifier to the dynamic dialect
- Lowering emits the verified allocation op (single-qubit and qreg(n))
- OpenQASM reifier recognizes allocation by op name, not generic shape
- Update lit/integration fixtures and visualization goldens
- Update test helpers and assertions to use the verified alloc op
