fix: add context,nestingLevel to toExternalHTML

Summary

This PR adds an extra parameter, nestingLevel to toExternalHTML. This can be helpful to render different styling / markup depending on a child nestinglevel.

I'm fine with nestingLevel, but would be wary of adding anything more complex like the parent block.
