fix: make stale adapter cleanup best effort

Summary

• stop stale adapter override cleanup from failing GetStatus and other refresh-driven control actions when the local config rewrite fails
• keep the existing cleanup behavior when the rewrite succeeds, and log a warning instead when it does not
• add a regression test that forces the config rewrite path to fail and verifies status refresh still succeeds
