fix(planning): repoint change links to flat 2.0.0 files (#94)

The 2.0.0 flatten renamed changes/<slug>/{design,change,plan}.md to changes/<slug>.md; repoint doc links to the flat targets, only where the flat file exists. Pre-existing active/archive dead links left untouched.
