The RenamePlan doc worked a little too well: the rename tooling rewrote the Life OS token inside the page's own title, so the arrow now points "LifeOS → LifeOS" — a name being renamed to itself. The plan achieved sentience and ate its own example.

You can tell it was originally "Life OS → LifeOS" because the Status table still says the scope is to retire Life OS and keep LifeOS.
Suggestion: exclude the doc's own title/callout context from RenameMap.json substitution so a page about a rename can still quote the name it's retiring.
